Introduction: Providing conventional, restorative dental care to children with special healthcare needs (CSHCN) often requires sedation using general anesthesia. Saliva consistency, diet, and oral hygiene practice are different for CSHCN, and limited evidence is available on the efficacy of silver fluoride (SF) for the management of carious lesions for this vulnerable population. Methods: Parents of CSHCN were educated about silver fluoride as a treatment option for caries. In total, 550 carious lesions from 100 participants were identified and scored according to the Nyvad Caries criteria. A total of 100 lesions with Nyvad scores 1, 2, and 3 were treated with a single application of silver fluoride and observed postoperatively at 1, 3, and 6 weeks. Result: The results indicate statistically significant (p < 0.05) differences in lesion remineralization over the 6-week follow-up period. At the 6-week follow-up, more than 85% of all lesions were remineralized across all children, regardless of condition or original Nyvad score of 1, 2, or 3. Conclusion: A single application of silver fluoride has demonstrated effectiveness in remineralization and inactivation of carious lesions over 6 weeks among Brazilian CSHCN. Silver fluoride should be considered an option for the management of carious lesions among CSHCN. Further studies are recommended, including larger sample sizes, longer follow-up times, a second application of SF, and different special needs conditions.

La diabetes mellitus tipo 2 (DMT2) es una enfermedad caracterizada por un estado inflamatorio crónico en la que algunos marcadores como las concentraciones de lípidos se han encontrado frecuentemente alterados. Se han descrito otros marcadores de inflamación alterados como las interleucinas 6, 8 y el factor de necrosis tumoral, sin embargo, existe poca información acerca del comportamiento del índice neutrófilo/linfocito (INL), la relación plaquetas/linfocitos (RPL) y el índice monocitos/linfocitos (IML), los coeficientes lipídicos y el índice aterogénico en pacientes con DMT2. Objetivo: Describir los parámetros aterogénicos y de inflamación en un grupo de pacientes con DMT2. Materiales y métodos: Se incluyeron y analizaron los antecedentes clínicos (antecedentes de la enfermedad, comorbilidad, tabaquismo y otras variables relevantes), así como parámetros hematológicos, bioquímicos y antropométricos de 42 pacientes, y se calcularon y evaluaron los coeficientes aterogénicos e índices de inflamación. Resultados: Se encontraron concentraciones elevadas de citocinas proinflamatorias IL-6 e IL-8, factor de necrosis tumoral-α y elevado INL. El 88% de los pacientes fueron clasificados como de alto riesgo de acuerdo con el índice aterogénico. Los pacientes exfumadores exhibieron niveles menores de IL-8 y niveles más altos de INL comparados con los que nunca han fumado. Conclusión: La evaluación de marcadores aterogénicos y de inflamación tales como el índice aterogénico, IL-8 y la INL permiten identificar a un subgrupo de pacientes con un alto riesgo de complicaciones graves y mortalidad. The Lake Titicaca frog is endangered due to threats such as water pollution, introduced species, and overharversting for markets, where people consume them as frog juice. This study, conducted June to November 2012, aimed to determinate the bacteria microflora living on the skin of frogs confiscated from the La Parada market, Lima, Peru, and housed individually in the Laboratory of Wildlife at the Faculty of Veterinary Medicine and Zootechnic of the Universidad Peruana Cayetano Heredia in Lima, Peru. Samples collected with sterile swabs and cultured on blood, tryptic soy, and MacConkey agars were investigated using commercially available test kits, to investigate the commonly encountered bacterial and potentially zoonotic microorganisms associated with their consumption. We found three species of zoonotic concern in the genus Vibro: Vibrio alginolyticus, Vibro cholerae, and Vibro fluvialis. Other Gram-negative species cultured included two different colonies of Aeromonas hydrophila, or Aeromonas caviae or Aeromonas sobria; Pseudomona luteola; one example of Weeksella virosa or Empedobacter brevis; and Citrobacter freundii. Gram-positive bacteria detected were Staphylococcus spp., Micrococcus spp., and Erysipelothrix rhusiopathiae. We recommend against the consumption of this frog due to the pathogens it may carry that could cause serious illness among consumers and in vendors who handle animals.

Objective: The aim of this case report is to describe the surgical removal of a mucocele and its histological analysis, in a child. Case Report: An 11-year-old female patient attended the Pediatric Dentistry clinic complaining of a lower lip lesion. During the anamnesis, the mother reported that the child had a habit of biting and sucking the spot frequently. Clinical examination showed the lesion was compatible with a mucocele. The proposed treatment was a complete enucleation of the lesion under local anesthesia. The incision and tissue divulsion were performed for maximum preservation of the mucosa, avoiding a possible recurrence. Total adjacent glands removal was also performed. The lesion was placed in 10% formaldehyde for histopathological analysis (H&E Staining), which showed dense connective tissue presenting chronic inflammatory infiltrate and extravasated mucin, presence of granulation tissue delimiting the area of extravasated mucin and presence of minor salivary glands. The patient was advised to quit the habit, and after seven days the sutures were removed. At the one-year follow-up there was no recurrence of the lesion. Conclusion: The proposed treatment proved to be effective without recurrence of the lesion.


Objetivo: El objetivo de este reporte de caso es describir la extirpación quirúrgica de un mucocele y su análisis histológico en un niño. Informe del caso: una paciente de 11 años de edad asistió a la clínica de Odontopediatria quejándose de una lesión en el labio inferior. Durante la anamnesis, la madre informó que el niño tenía la costumbre de morder y chupar el lugar con frecuencia. En el examen clínico, la lesión fue compatible con un mucocele. El tratamiento propuesto fue una enucleación completa de la lesión bajo anestesia local. La incisión y la divulgación del tejido se realizaron para la máxima preservación de la mucosa, evitando una posible recurrencia. También se realizó la extracción total de las glándulas adyacentes. La lesión se colocó en formaldehído al 10% para el análisis histopatológico (tinción H&E), que mostró tejido conectivo denso que presenta infiltrado inflamatorio crónico y mucina extravasada, presencia de tejido de granulación que delimita el área de mucina extravasada y presencia de glándulas salivales menores. Se aconsejó al paciente que abandonara el hábito, y después de siete días se retiraron las suturas. En el seguimiento de un año no hubo recurrencia de la lesión. Conclusión: El tratamiento propuesto demostró ser efectivo sin recurrencia de la lesión.

The Peruvian Andes are the home of 27 species of frogs of the genus Telmatobius, many of which are critically endangered. Illegal trade of adult frogs for purported medical properties likely represents the major threat facing these species. This activity, besides reducing their populations, may contribute to the dissemination of the fungus Batrachochytrium dendrobatidis (Bd), which causes chytridiomycosis, a disease posing a threat to many populations of amphibians. We screened frogs confiscated by the Administration of Forestry and Wildlife in Lima, Peru, for Bd. We used real-time PCR to diagnose Bd at the Laboratory of Wildlife, Faculty of Veterinary Medicine and Zootecnics, Universidad Peruana Cayetano Heredia, in Lima and Pisces Molecular Laboratory in Boulder, Colorado, US. Of 62 samples collected during this study, 60% (37) were PCR positive for Bd, confirming that illegal trade of amphibians can pose a risk for disseminating Bd.

RESUMO

The Lake Titicaca frog (Telmatobius culeus) is critically endangered, primarily from overexploitation. However, additional threats, such as chytrid fungus ( Batrachochytrium dendrobatidis ), are poorly studied. We found moderate levels of chytrid infection using quantitative PCR. Our results enhance our understanding of chytrid tolerance to high pH and low water temperature.

Identificar conocimientos que poseen los pobladores de la cuenca del río Ucayali sobre la biología, conservación y problemática, asi como avistamientos previos del manatí amazónico Trichechus inunguis. Materiales y métodos: Se encuestaron 421 personas de diferentes edades, sexos y ocupaciones, en diferentes localidades de la cuenca del río Ucayali. Resultados: El 88.8% reconoció a la especie, y el 80.1% conocen al manatí con el nombre de vaca marina. El 65.1% declaran haber consumido su carne de manera poco frecuente y oportunista. La principal razón que justifican para capturarlos fue su buen sabor (46.4%). Así mismo, 51.9% de los encuestados afirmó saber que su caza es ilegal y el 24% refirió haberlo avistado en el área de estudio. Asimismo, 96.7% manifestaron interés para que esta especie animal se conserve. Conclusiones: Los conocimientos de los pobladores encuestados son insuficientes sobre la situación poblacional, las amenazas y la legislación actual que rige la caza del manatí. Actualmente, con poca frecuencia, se siguen avistando ejemplares en vida silvestre dentro del área de estudio, pero también se sigue dando su captura. Se recomienda incluir la educación ambiental en estas comunidades como principal herramienta para la conservación del manatí amazónico...


To collect information about the knowledge of the inhabitants from the Ucayali River basin about the amazonian manateeÆs, Trichechus inunguis, biology, conservation and problematic as well as previous sightings. Materials and methods: 421 people were surveyed without bias of age, sex or occupation, in different localities of the Ucayali River basin. Results: 88.8% of the interviewed recognized species, most of them referring to it as ôsea cowõ (80.1%). The principal use attributed to this animal is the consumption of its meat (65.1%). Also 51.9% of the surveyed are aware that hunting is illegal. According to the surveyed, the consumption of this species is rare and opportunistic, being the good taste of its meat (46.4%) the main reason for its capture. There is a great interest from behalf the population in the conservation of this animal (96.7%). As for the sightings, 24.0% were within the study area. Conclusions: There is a lack of knowledge from the surveyed on the current population, threats and the laws regulating the hunting of the amazonian manatee. Currently, with low frequency, sightings are still occurring within the study area, but also their capture. Environmental education is the main tool required for the conservation of the amazonian manatee in these communities...

Introducción: las enfermedades cardiovasculares constituyeron en 2004 la causa más frecuente de muerte en Argentina,duplicando a las neoplasias malignas en su conjunto. El accidente cerebrovascular (ACV) representó el7,5% del total de muertes. La Asociación Americana del Corazón propone el empleo de la escala prehospitalaria delaccidente cerebrovascular de Cincinnati (CPSS) y la escala de evaluación prehospitalaria del ACV de Los Ángeles(LAPSS) para evaluar sujetos con posible ACV.Objetivos: evaluar el conocimiento de estas escalas por médicos de guardia, y su opinión respecto al valor que ellaspodrían tener sobre su práctica.Material y método: estudio prospectivo y observacional encuestando 569 médicos durante diciembre de 2008 amarzo 2009 en distintas ciudades de las provincias de Santa Fe, Entre Ríos y Chaco, Argentina.Resultados: un 32,3% manifestó conocer las escalas, pero solamente 8,4% de ellos pudo enumerar los puntos deCPSS, un 1,9% pudo hacerlo con LAPSS y el 0,7% ambas. Quienes recordaban una o ambas escalas manifestaronsentirse más seguros al evaluar un posible ACV (p= 0,02); ninguno de los que recordaban LAPSS manifestó sentirseinseguro. Quienes se sentían menos seguros fueron los que atribuyeron más beneficios a las escalas (p= 0,0003).Los no especialistas las consideraron más útiles (p= 0,005). El 89% refirió que las escalas beneficiarían su práctica.Conclusiones: ambas escalas fueron consideradas de valor por médicos de guardia para evaluar pacientes con posibleACV. Cerebrovascular disease caused 7.5% of total deaths. The American Heart Association proposes the use of the Cincinnati Prehospital Stroke Scale (CPSS) and Los Angeles Prehospital Stroke Screen (LAPSS) toevaluate patients with possible cerebrovascular disease. Objective: To evaluate the awareness of these scales knowledge among doctors working in emergency rooms and prehospitalconsultation, and what are their opinions about the usefulness of the scales.Material and method: Observational and prospective survey of 569 doctors from the Argentinian provinces of Santa Fe,Entre Rios, and Chaco from December 2008 to March 2009. Results: One third (32.3%) of doctors included in the mentioned survey declared to know the scales, although only 8.4% of them could enumerate CPSS points, 1.9% could enumerate LAPSS points and 0.7% remembered both CPSS andLAPSS points. Those who remembered either one or both scales felt safer evaluating a possible stroke (p= 0.02) while and no doctor who remembered LAPSS felt doubtful. Those who felt more hesitant where the doctors who adscribed more benefits to the scales (p= 0.0003). Non specialists considered scales more useful (p= 0.005). Overall, 89% of doctors admitted scales would benefit their practice. Conclusion: Both scales were highly valued by doctors on call in order to diagnose patients with possible stroke. Describir las características del decomiso de animales silvestres y productos derivados realizado por la Administración Técnica Forestal y de Fauna Silvestre (ATFFS) durante el período 2000 - 2007 en la ciudad de Lima - Perú. Material y métodos: Para ello, se revisó la base de datos de decomisos de la ATFFS para el período de estudio. Resultados: se encontró 17 932 animales vivos y 81 564 productos derivados decomisados en el período 2000-2007. Entre los animales vivos, el 47,3% (8479) fueron aves, 34,1% (6111) anfibios, 13,1% (2341) reptiles y 5,6% (1001) mamíferos. La mayor cantidad de especies decomisadas corresponde a las aves de la familia Psittacidae. De los productos derivados de animales silvestres, el 98,3% (80216) correspondió a animales taxidermizados, 0,8% (627) pieles, 0,7% (600) instrumentos y 0,1% (121) carnes. También se encontró ejemplares no identificados lo que no permite obtener información necesaria para la evaluación de la situación actual de las especies sujetas a tráfico ilegal. Conclusiones: Debido a la magnitud del tráfico de animales silvestres y productos derivados se hace necesario buscar nuevas alternativas para reducir este tipo de actividad ilícita que afecta a la biodiversidad del país...


Objectives: To describe the characteristics of the confiscation wildlife and products made by the Technical Administration of Forestry and Wildlife (ATFFS) during the period from 2000 to 2007 in the city of Lima - Peru. Methods: To do this, we reviewed the database ATFFS for the study period Results: 17932 live animals and 81564 products confiscated in the period 2000-2007. Among living animals, 47.3% (8479) were birds, 34.1% (6111) amphibian, 13.1% (2341) Reptiles and 5,6% (1001) mammals. Most species of birds confiscated corresponds to family Psittacidae. Of wildlife products, 98.3% (80216) corresponded to animal taxidermy, 0.8% (627) furs, 0.7% (600) instruments and 0.1% (121) bush meat. Also found an unidentified specimen which does not allow obtaining information necessary for the evaluation of the current status of the species subject to trade. Conclusions: Because of the extent of trade of wildlife and their products is necessary to find new ways to reduce this kind of illegal activity affecting the countryÆs biodiversity...

RESUMO

En el presente estudio se cuantificó la producción de biopelículas individuales y mixtas de Candida albicans y Staphylococcus aureus para determinar si dichas biopelículas mixtas se favorecen sinérgicamente. Los ensayos se realizaron utilizando placas de microtitulación de poliestireno de 96 pocillos de fondo plano, se determinó la actividad metabólica de las células en la biopelícula por medio de la reducción enzimática de una sal de tetrazolio (XTT) a través de los cambios colorimétricos que fueron medidos a 490nm. Para visualizar las biopelículas de cada microorganismo y su cinética de crecimiento se utilizo microscopia láser confocal. La mayor formación de biopelícula se observó en las biopelículas mixtas, seguida de las de Candida albicans y, por último, la menor producción la obtuvo Staphylococcus aureus, lo cual nos sugiere la presencia de una relación sinérgica entre los microorganismos ensayados(AU)

El Granuloma Piogénico (GP) es una lesión no neoplásica de la cavidad bucal y de la piel relativamente poco frecuente y extremadamente rara en el tracto gastrointestinal. En la cavidad bucal donde es a menudo encontrada sobre tejido queratinizado. Se considera una hiperplasia inflamatoria. Clínicamente es una lesión uniforme o exofítica lobulada, pequeña, pápulas rojas eritematosas sobre una base pediculada o a veces sésil y que habitualmente sangra. El diámetro varía de pocos milímetros a varios centímetros. El propósito de este artículo es presentar un caso de Granuloma Piogénico, y valorar los factores que intervienen en su patogenia, así como la clínica, diagnóstico diferencial y tratamiento


Pyogenic granuloma (PG) is a non-neoplastic tumor lesion of the bucal cavity and skin relatively rare and extremely rare in the gastrointestinal tract. In the bucal cavity where it is often found on keratinized tissue. It is considered an inflammatory hyperplasia. Clinically it is an uniform or lobulated exophytic lesion, small, red erythematous papules on a pedunculated or sometimes sessile base that usually bleed. The diameter varies from a few millimeters to several centimeters. The purpose of this paper is to present a case of pyogenic granuloma, and evaluate the factors involved in its pathogenesis and the clinical differential diagnosis and treatment

RESUMO

The 2010 undergraduate medical degree curriculum at the National Autonomous University of Mexico (UNAM) faculty of medicine is based on the reality of the operating structure of the medical care units qualified for teaching. The clinical teaching/learning is based on the cooperative work of the professor and student; this means, it is based on the institutional physician responsible for medical care in a professor/assistant action together with the student being brought up. Therefore, the permanent education and training of all medical teams in the institutions offering teaching is essential. Teaching must be one distinctive characteristic of excellence for the units of the Health Ministry as it is teaching the central factor that raises the quality of medical care. The clinical evaluation must be permanent, improving the value of the daily action in front of a patient at the formative level and as a means to allow the assessment for its development, as it is the clinical aspect that is the essence of medicine.

El incremento en las últimas dos décadas en la incidencia de fungemias causadas por especies levaduriformes en pacientes inmunodeprimidos susceptibles y la poca sensibilidad del cultivo de sangre convencional hacen necesario el desarrollo de enfoques alternativos para la detección temprana y la identificación de las especies responsables. El objetivo de este trabajo ha sido comparar la utilidad de la prueba molecular de la reacción en cadena de la polimerasa (PCR) y métodos convencionales para identificar aislamientos clínicos de diferentes especies, incluyendo el sistema ATB ID32C (bioMérièux, Francia), el cultivo cromogénico Chromagar Candida® (Chromagar, Francia) y la morfogénesis en agar harina de maíz. Se estudiaron 79 aislamientos clínicos en los cuales la especie más prevalente usando el sistema ATB ID32C y la PCR fue C. albicans, seguida por C. tropicalis, C. glabrata y C. krusei. Los patrones de PCR obtenidos para la identificación de aislamientos clínicos fueron estables y consistentes en los diferentes ensayos independientes y mostraron una buena reproducibilidad. Se concluye que la PCR con los cebadores específicos para cada especie, que amplifican los genes ITS1 e ITS2 del ARNr o del gen de la topoisomerasa II, demostró ser un método sensible y específico para la identificación de los aislamientos de C. glabrata C. krusei, C. albicans y, con menor especificidad, para C. tropicalis(AU)

In our earlier paper we found that esophageal cancer was the most frequent localization among all the digestive tract cancers. Furthermore, in four departments belonging to our province we also observed a colon-esophagus relationship of virtually 1 to 1. In this study we aimed to: 1) estimate the prevalence of esophagus and stomach tumors: a) in the Endoscopy Department, b) in all the hospital, comparing it with colon cancer and, c) as causes of death in our region of influence, in relation to all the neoplastic pathologies. 2) analyze the histological lineage and the anatomical location, according to sex, age and origin. We evaluated: 3,396 upper digestive tract endoscopies, the diagnoses of digestive tract cancer from the Histopathology Service, the discharges from hospital as the result of digestive tract cancer from the General Archives, all of them from Cullen Hospital, and death register of the province of Santa Fe accounting for digestive tract cancer. A descriptive analysis was carried out and spreadsheet from the Open Office 2.2 version and SPSS 10.0 version were used. We observed that esophageal cancer is still very frequent in the Endoscopy Department and in the Histopathology Service but not in the diagnoses at discharge from hospital. It shares similar epidemiological features to the ones observed in the international literature. The reduction of the relationship between colon and esophageal cancer in the global mortality rate of the country turns out to be surprising. In three departments situated on the north of the province it was observed a colon-esophagus cancer relationship of 1:1.

RESUMO

We present a historical framework for the initiation and development of obstetrical-gynecological (OB-Gyn) surgery during the 19th century in Mexico. The importance of a new vision is underlined, and the founders of medical science, the Romanticist surgeons who initiated the teaching of modern medicine and surgery, supported the change. The French influenced the advent of anesthesia, asepsis, antisepsis and transfusion, which has a major significance for the accomplishment of OB-Gyn Surgery. Important persons in this field of medicine are mentioned and we conclude with techniques of contemporary surgery in the next century, integrated with medical care.
